Please complete this form to reserve High Holiday Childcare (Infant to TK) and/or High Holiday Experience (Grades K-7).

For ALL children attending:

• Parents must be in services on campus to utilize Day Care service.
• Please bring a dairy/parve and nut free lunch.

For High Holiday Experience Students:

• Write the first and last name and grade level on the bag.
• Each child should have their own lunch because grade levels eat separately.
• Snacks will be provided.
